The Fund for Public Health in New York City, on behalf of the New York City Department of Health and Mental Hygiene, is inviting vendors to submit proposals to provide a software-as-a-service (SaaS) solution to support evidence-based interventions (EBI) aimed at the prevention and management of chronic diseases. The selected SaaS solution will provide electronic management of EBI class delivery and administration; facilitate referrals and corresponding referral status feedback; and allow de-identified population level tracking of EBI programming across the city.
