The Fund for Public Health in New York City (FPHNYC) in collaboration with the New York City Department of Health and Mental Hygiene (DOHMH), is seeking proposals from community organizations to focus on community-wide prevention of hypertension and/or its risk factors through only one of the following areas:

  • Encouraging and Promoting Physical Activity Opportunities
  • Hypertension Prevention and Control
  • Promoting Healthy Food Access and Consumption
